医局長挨拶

2015年9月より宮崎大学に消化器内科学講座が設立され、2017年4月より講座再編が行われました。2016年4月に教授として北海道大学より河上洋教授が就任され、胆膵疾患における最新の超音波内視鏡関連手技を自ら先頭に立ち、大学病院の臨床の現場で厳しくご指導いただいております。また臨床のみならず、多施設共同無作為化比較試などの臨床研究への参加や、若手医師への英語論文の投稿なども積極的に行っており、大学病院の臨床医として皆様に還元できるような診療を心がけております。

2025年4月現在ですが、河上洋教授、永田賢治准教授をはじめ、教員16名、医員12名とともに大学病院にて診療・教育・研究を行っております。協力関連病院としては、県立延岡病院、県立宮崎病院、古賀総合病院、JCHO宮崎江南病院、都城医療センター、平和台病院などに医師を派遣しており、地域医療に貢献しています。

炎症性腸疾患に対して診断のために小腸内視鏡やカプセル内視鏡などの特殊検査を積極的に行い、治療として血球除去療法や免疫抑制剤、生物学的製剤を行っています。早期消化管癌に対して最新の特殊光を用いた拡大内視鏡診断や、治療として内視鏡的粘膜下層剥離術（ESD）、手術不能の進行癌に対して最善の化学療法の選択および放射線治療から緩和医療まで行っています。胆膵疾患に対して超音波内視鏡（EUS）、超音波内視鏡下穿刺吸引法（EUS-FNA）、内視鏡的逆行性胆管膵管造影（ERCP）、またERCP困難例に対しては積極的にinterventional EUSも行っています。肝疾患では肝細胞癌に対し肝動脈化学塞栓療法、ラジオ波焼灼療法、分子標的薬、免疫チェックポイント阻害薬などによる集学的な集学的治療を行い、食道胃静脈瘤に対して内視鏡的治療、カテーテル治療（IVR）、慢性肝疾患や肝腫瘍に対して超音波ガイド下肝生検などを行っています。また消化管出血などの救急疾患に対して救命救急センターとの連携により緊急対応を行っています。

医療の細分化に伴い、診療は煩雑さを増しているにもかかわらず、働き方改革や専門医制度の見直しなどのさまざまな問題が重なり、多事多端ではございますが、常に志を高く、前を向いて努力していきたいと考えております。診療、研究、教育に全力を尽くし、宮崎の最後の砦である宮崎大学病院を守り抜き、皆様から信頼される消化器内科専門施設を目指していきます。

Gastroenterology

Overview

The Department of Gastroenterology at University of Miyazaki was established in September 2015, with Professor Kawakami Hiroshi appointed as the founding chair in April 2016 after his tenure at Hokkaido University. Under his leadership, the department has become a center of excellence in the diagnosis and treatment of pancreatobiliary diseases, particularly through the advanced use of state-of-the-art endoscopic ultrasound–guided techniques. Professor Kawakami leads the clinical team at the forefront of patient care, providing expert instruction and mentorship within the university hospital.

Beyond clinical excellence, the department is deeply committed to academic advancement. We actively engage in multicenter randomized controlled trials and other collaborative clinical studies, while fostering an environment that supports young physicians in publishing high-quality research in international journals. Through the integration of cutting-edge clinical practice, research, and education, we aim to deliver innovative and reliable medical care that contributes meaningfully to the regional community and the global medical field.

Advanced Clinical Care & Research

1. Inflammatory Bowel Disease

We actively perform advanced diagnostic procedures, including balloon-assisted enteroscopy and capsule endoscopy, to evaluate small-bowel lesions. Our therapeutic approaches include granulocyte and monocyte apheresis, immunosuppressive agents, and biologic therapies, tailored to each patient’s disease activity and condition.

2. Early Gastrointestinal Cancer

We utilize state-of-the-art magnifying endoscopy with image-enhanced technologies for precise diagnosis. Therapeutic options include endoscopic submucosal dissection (ESD). For patients with unresectable advanced cancer, we provide optimal systemic chemotherapy, radiotherapy, and comprehensive palliative care.

3. Pancreatobiliary Diseases

Our department offers advanced pancreatobiliary diagnostics and interventions, including endoscopic ultrasound (EUS), EUS-guided fine-needle aspiration (EUS-FNA), and endoscopic retrograde cholangiopancreatography (ERCP). For cases in which ERCP is technically challenging, we actively perform interventional EUS procedures.

4. Liver Diseases

For hepatocellular carcinoma, we provide multidisciplinary treatment approaches, including transarterial chemoembolization (TACE), radiofrequency ablation (RFA), molecular targeted therapies, and immune checkpoint inhibitors. We also offer endoscopic and interventional radiology treatments for esophagogastric varices, as well as ultrasound-guided liver biopsy for chronic liver diseases and hepatic tumors.

5. Emergency Care

In close collaboration with the Emergency and Critical Care Center, we provide prompt and effective management of gastrointestinal emergencies, including acute gastrointestinal bleeding.
